What Is Atrial Fibrillation?

Atrial fibrillation (AFib) is the most common type of irregular heartbeat. Instead of beating in a regular, steady rhythm, the upper chambers of your heart (the atria) quiver or "fibrillate" chaotically. This causes your heart to beat irregularly and often too fast.

Normal Heart Rhythm vs. AFib:

Rate: 60-100 beats per minute, steady and predictable

Why AFib Matters: Stroke Risk

The biggest danger of AFib is stroke. When your heart doesn't beat properly, blood can pool in the upper chambers and form clots. If a clot breaks loose and travels to your brain, it causes a stroke.

AFib increases stroke risk by 5 times. About 1 in 5 strokes is caused by AFib.

AFib Affects 6-12 Million Americans

  • Risk increases with age (9% of people over 65 have AFib)
  • More common in men than women
  • Caused by high blood pressure, heart disease, sleep apnea, obesity, alcohol
  • Can be temporary or permanent
Recognizing AFib Symptoms

Some people with AFib feel nothing at all. Others notice obvious symptoms:

Important Notes:

  • Asymptomatic AFib: About 1 in 4 people with AFib have no symptoms at all
  • Paroxysmal AFib: Comes and goes (episodes last less than 7 days)
  • Persistent AFib: Lasts longer than 7 days
  • Permanent AFib: Continuous, doesn't go away

Call 911 Immediately If You Experience:

  • Sudden severe chest pain
  • Sudden weakness, numbness, or drooping on one side of face or body
  • Sudden trouble speaking or understanding speech
  • Sudden vision problems
  • Sudden severe headache with no known cause
  • These could be signs of a stroke!
Assessing Your Stroke Risk: CHA₂DS₂-VASc Score

Doctors use the CHA₂DS₂-VASc score to estimate your stroke risk and determine if you need blood thinners (anticoagulation):

What Your Score Means:

Score 0 (men) or 1 (women with no other risk factors)

Low Risk: Anticoagulation not recommended (or aspirin)

Score 1 (men) or 2 (women)

Moderate Risk: Consider anticoagulation (discuss with doctor)

Score ≥2 (men) or ≥3 (women)

High Risk: Anticoagulation strongly recommended

Blood Thinners (Anticoagulation): Your Stroke Protection

Blood thinners reduce stroke risk by 60-70%!

Anticoagulants (blood thinners) are the most important treatment for AFib. They prevent blood clots from forming in your heart, dramatically reducing your stroke risk.

Types of Blood Thinners:

Warfarin (Coumadin) - The Original

Pros: Decades of proven safety, reversible, covered by insurance

Cons: Requires regular blood tests (INR monitoring), many food and drug interactions, dose adjustments

Diet: Need to keep vitamin K intake consistent (leafy greens)

Target INR: 2.0-3.0 (measures how thin your blood is)

DOACs (Direct Oral Anticoagulants) - Newer Options

Examples: Apixaban (Eliquis), rivaroxaban (Xarelto), edoxaban (Savaysa), dabigatran (Pradaxa)

Pros: No regular blood tests needed, fewer food/drug interactions, predictable dosing

Cons: More expensive, requires good kidney function, must take exactly as prescribed

Most popular: Apixaban (Eliquis) - taken twice daily, safest bleeding profile

Critical Blood Thinner Safety:

  • Never skip doses - even one missed dose increases stroke risk
  • Don't stop without doctor approval - stroke risk rebounds within days
  • Tell ALL healthcare providers you're on blood thinners (dentist, surgeon, etc.)
  • Watch for signs of bleeding (blood in stool/urine, easy bruising, prolonged bleeding)
  • Avoid NSAIDs (ibuprofen, naproxen) - use acetaminophen (Tylenol) instead
  • Be cautious with falls - wear sturdy shoes, remove tripping hazards

What About Aspirin?

Aspirin is NOT adequate for AFib stroke prevention. Studies show aspirin reduces stroke risk by only 20% (compared to 60-70% with anticoagulants). If you have AFib and stroke risk factors, you need a real blood thinner, not aspirin.

Managing Your Heart Rhythm: Rate Control vs. Rhythm Control

Beyond blood thinners, there are two main strategies to manage AFib:

Rate Control Strategy

Goal: Let your heart stay in AFib, but slow it down to a normal rate (60-100 bpm at rest)

Who: Older patients, minimal symptoms, permanent AFib

Medications:

  • Beta blockers: Metoprolol, atenolol, carvedilol
  • Calcium channel blockers: Diltiazem, verapamil
  • Digoxin: Less commonly used

Rhythm Control Strategy

Goal: Try to restore and maintain normal sinus rhythm

Who: Younger patients, symptomatic AFib, new-onset AFib

Medications:

  • Flecainide, propafenone: "Pill-in-pocket" for paroxysmal AFib
  • Amiodarone: Most effective, but has side effects
  • Sotalol, dofetilide: Require monitoring

Important:

You still need blood thinners even if you're in normal rhythm! AFib can come back at any time, and stroke risk remains high.

Catheter Ablation: A Potential Cure

Catheter ablation is a procedure that can potentially cure AFib by destroying the abnormal heart tissue causing the irregular rhythm.

How Ablation Works:

  1. Cardiologist threads thin catheters through veins to your heart
  2. Uses heat (radiofrequency) or cold (cryoablation) to create scar tissue
  3. Scars block the abnormal electrical signals causing AFib
  4. Procedure takes 3-6 hours, usually requires overnight hospital stay

Who Should Consider Ablation?

  • Symptomatic paroxysmal (intermittent) AFib despite medications
  • Younger patients who want to avoid lifelong medications
  • AFib causing heart failure (weakening the heart)
  • Preference for procedure over long-term medications

Success Rates:

  • Paroxysmal AFib: 60-80% success rate (free from AFib at 1 year)
  • Persistent AFib: 40-60% success rate
  • May require repeat procedures (20-40% need a second ablation)
  • Success higher when done early, before AFib becomes permanent

Risks:

  • Bleeding, infection (1-2%)
  • Blood clots, stroke (<1%)
  • Heart perforation (rare, <1%)
  • Damage to esophagus or phrenic nerve (very rare)

Limit Alcohol

Even moderate drinking can trigger AFib ("holiday heart syndrome"). Limit to 1 drink/day or avoid entirely.

Lose Weight

Obesity increases AFib risk. Losing 10% of body weight can reduce AFib episodes significantly.

High BP is the #1 risk factor for AFib. Target <120/80 mmHg.

Treat Sleep Apnea

Sleep apnea makes AFib worse and reduces ablation success. Use CPAP if prescribed.
