Treat dyslipidemia (abnormal cholesterol) earlier in life, use more modern risk calculators, aim for specific LDL-C targets, and take advantage of new medications when lifestyle changes and statins alone aren't enough. The emphasis is on reducing lifelong exposure to cholesterol that damages your arteries.

1. Start Young: Earlier Screening for Everyone

The guidelines strongly emphasize that cholesterol problems should be caught early in life, because the longer your arteries are exposed to high cholesterol, the greater your risk of heart attack and stroke.

Updated Screening Recommendations:

Children Ages 9-11

All children should be screened for cholesterol with a lipid profile between ages 9 and 11 to catch inherited conditions like familial hypercholesterolemia (FH), which affects 1 in 250 people.

Children with Family History

If a close relative has premature heart disease, very high cholesterol, or FH, screening should start as early as age 2.

Adults Starting at Age 19

All adults should begin regular lipid screening at age 19 and repeat at least every 5 years, with more frequent testing if you have additional risk factors.

Young Adults with Long-Term Risk

If you're between 30 and 59 years old with low 10-year risk (<3%) but your 30-year ASCVD risk is ≥10%, or your LDL-C is 160-189 mg/dL, the guidelines say it's reasonable to discuss starting a moderate-intensity statin. The idea is that decades of even mildly elevated cholesterol cause significant cumulative artery damage.

3. Know Your Goal: Specific LDL-C Targets Are Back

A major change in the 2026 guidelines is the return of specific LDL-C and non-HDL-C treatment goals. Instead of just aiming for a percentage reduction, your doctor now has a specific number to target based on your risk level.

Bottom line: Lower is better when it comes to LDL-C. The benefits of keeping LDL-C low grow over time, which is why the guidelines emphasize starting treatment earlier and maintaining it consistently.

4. New Tests to Know About: ApoB and Lp(a)

The 2026 guidelines place new emphasis on two additional blood tests beyond the standard lipid panel:

Apolipoprotein B (ApoB)

ApoB directly measures the number of "bad" cholesterol particles in your blood. Sometimes your LDL-C number looks fine, but you actually have too many harmful particles. This is especially common in people with diabetes, CKM syndrome, or high triglycerides.

When to test: After you've reached your LDL-C goal, measuring ApoB can help determine if you need additional therapy.

Lipoprotein(a) [Lp(a)]

Lp(a) is a genetically determined risk factor. Levels above 125 nmol/L (50 mg/dL) are associated with increased heart disease risk. Since Lp(a) levels don't change much over your lifetime, a single measurement is usually sufficient.

New recommendation: All adults should have Lp(a) measured at least once for ASCVD risk assessment. Elevated Lp(a) is considered a risk enhancer and may prompt earlier or more aggressive treatment.

5. Medications: A Broader Toolkit

Statins remain the cornerstone of cholesterol treatment, but the 2026 guidelines emphasize that combination therapy can achieve much lower LDL-C levels when needed.

Medication Tiers:

First Line: Statins

High-intensity statins (atorvastatin 40-80 mg, rosuvastatin 20-40 mg) lower LDL-C by ≥50%. Moderate-intensity statins lower LDL-C by 30-49%. Statins are safe, effective, and backed by decades of clinical trial evidence.

Second Line: Ezetimibe — Add It Earlier

Can be added to a statin to lower LDL-C by an additional 18-25%. Key change: The 2026 guidelines recommend adding ezetimibe as soon as you're not at your LDL-C goal on a statin — you no longer need to maximize the statin dose first. This is important because statin side effects are dose-dependent, so combining a moderate-dose statin with ezetimibe can be better tolerated than a high-dose statin alone. Third Line: PCSK9 Inhibitors & Bempedoic Acid

PCSK9 inhibitors (evolocumab, alirocumab) are injectable medications that lower LDL-C by 45-64%. Bempedoic acid is an oral option that lowers LDL-C by 17-24% and is especially useful for people who can't tolerate statins (it works in the liver, not muscles).

Newer Option: Inclisiran

A small-interfering RNA (siRNA) therapy given as an injection every 6 months that lowers LDL-C by 48-52%. An option for those who have trouble with more frequent injections.

What About Supplements?

The 2026 guidelines explicitly state that dietary supplements (fish oil, cinnamon, garlic, turmeric, plant sterols, red yeast rice) are not recommended for lowering LDL-C or triglycerides based on limited and inconsistent evidence. FDA-approved prescription medications are far more effective and well-studied.

6. Live Well: Lifelong Heart-Healthy Habits

Healthy lifestyle habits remain the foundation of all cholesterol treatment, at every risk level. The guidelines give healthy lifestyle a Class 1 (strongest) recommendation.

Heart-Healthy Diet

Emphasize fruits, vegetables, nuts, legumes, whole grains, and fiber. Replace saturated and trans fats with unsaturated fats (olive oil, fish, avocados). Mediterranean, DASH, and vegetarian diets have the strongest evidence.

Regular Physical Activity

At least 150 minutes/week of moderate-intensity aerobic exercise (or 75 minutes vigorous), plus 2 days/week of resistance exercise. Activity improves HDL-C, lowers triglycerides, and enhances insulin sensitivity.

Maintain a Healthy Weight

If overweight, a 5-10% weight loss can meaningfully improve lipid profiles. Weight management is especially important for lowering triglycerides.

Avoid Tobacco & Limit Alcohol

Smoking cessation improves HDL-C and overall cardiovascular health. Alcohol raises triglycerides and should be limited or eliminated, especially in those with elevated TG levels.

Even with genetic risk: Studies show that healthy lifestyle habits can reduce cardiovascular risk by approximately 50%, even in people who are genetically predisposed to heart disease.

7. Stay Consistent: Treatment Benefits Grow Over Time

The guidelines emphasize that cholesterol management is a lifelong commitment. Benefits accumulate the longer you maintain lower cholesterol levels.

Monitoring Schedule:

4-12 weeks after starting or changing medication: Recheck your lipid panel to assess response
Every 6-12 months thereafter: Continue monitoring to ensure you're at goal and adhering to therapy
Every 5 years (if not on medication): Routine lipid screening for adults with normal levels

Don't stop medications without consulting your doctor. The benefits of cholesterol-lowering therapy continue as long as you take the medication. Stopping statins has been associated with increased cardiovascular events.

The 2026 guidelines include expanded guidance for specific groups:

Diabetes

CAC = 0: Defer statin therapy, reassess in 3-7 years. Very low near-term risk.
CAC 1-99: LDL-C <100 mg/dL, non-HDL-C <130 mg/dL. Moderate-intensity statin.
CAC 100-299 (or ≥75th percentile): LDL-C <70 mg/dL, non-HDL-C <100 mg/dL. High-intensity statin.

Women & Reproductive Risk Markers

Early menopause (<45 years), preeclampsia, gestational diabetes, and other adverse pregnancy outcomes are now recognized as risk enhancers that should factor into ASCVD risk assessment.

Chronic Kidney Disease (CKD Stage 3+)

Adults ages 40-75 with CKD and elevated LDL-C are recommended moderate-intensity statin or statin + ezetimibe combination to reduce ASCVD risk.

People Living with HIV

Statin therapy (specifically pitavastatin) is recommended for ages 40-75 on stable antiretroviral therapy to reduce the elevated cardiovascular risk seen in this population.

Older Adults (≥75 years)

Treatment decisions should balance ASCVD prevention with patient priorities, functional status, life expectancy, and potential for medication side effects. A benefit-risk discussion is essential.
