The "Bad" Cholesterol - What You Need to Know

What Is LDL Cholesterol?

LDL (Low-Density Lipoprotein) cholesterol is often called "bad" cholesterol because high levels lead to plaque buildup in your arteries, increasing your risk of heart attack and stroke.

How LDL Damages Your Arteries:

They deposit cholesterol in artery walls, especially where there's damage or inflammation.

Over years, cholesterol deposits harden into plaque, narrowing arteries and reducing blood flow.

Plaque can rupture

A blood clot forms on the ruptured plaque, suddenly blocking blood flow - causing a heart attack or stroke.

The Good News: Lowering LDL cholesterol significantly reduces your risk of heart attack and stroke. For every 39 mg/dL reduction in LDL, your risk drops by about 20%.

What Should My LDL Cholesterol Be?

Your LDL target depends on your cardiovascular risk. Your risk level is calculated based on factors like your age, blood pressure, smoking status, diabetes, and history of heart disease—not your LDL number itself. Once we know your risk level, we can determine your LDL target. The higher your risk, the lower your LDL should be:

Very High Risk

LDL Target: < 55 mg/dL  |  Non-HDL Target: < 85 mg/dL

You're at very high risk if you have:

  • History of heart attack, stroke, or other cardiovascular disease (ASCVD)
  • Diabetes PLUS additional cardiovascular risk factors
  • Severe chronic kidney disease (eGFR < 30)
  • Familial hypercholesterolemia with ASCVD or additional risk factors
  • Coronary artery calcium (CAC) score ≥ 1000

High Risk

LDL Target: < 70 mg/dL  |  Non-HDL Target: < 100 mg/dL

You're at high risk if you have:

  • 10-year ASCVD risk ≥ 10% (by PREVENT calculator)
  • Diabetes (without other risk factors)
  • Chronic kidney disease stage 3+ (eGFR < 60)
  • LDL ≥ 190 mg/dL (severe hypercholesterolemia)
  • CAC score 100-999

Intermediate Risk

LDL Target: < 100 mg/dL  |  Non-HDL Target: < 130 mg/dL

You're at intermediate risk if you have:

  • 10-year ASCVD risk 5% to < 10% (by PREVENT calculator)
  • Multiple risk factors (high BP, smoking, family history)
  • Moderate- to high-intensity statin recommended

Borderline Risk

LDL Target: < 100 mg/dL  |  Non-HDL Target: < 130 mg/dL

You're at borderline risk if you have:

  • 10-year ASCVD risk 3% to < 5% (by PREVENT calculator)
  • Risk enhancers may tip the decision toward medication (e.g., elevated Lp(a), hsCRP ≥ 2 mg/L, family history)
  • CAC scoring can help guide the treatment decision

Low Risk

LDL Target: < 100 mg/dL  |  Non-HDL Target: < 130 mg/dL

You're at low risk if you have:

  • 10-year ASCVD risk < 3% (by PREVENT calculator)
  • No diabetes, heart disease, or major risk factors
  • Lifestyle changes are the main focus
  • If age 30-59 with LDL 160-189 or 30-year risk ≥ 10%, moderate statin may be reasonable

Important:

Your doctor will calculate your personalized cardiovascular risk using the PREVENT-ASCVD calculator (which replaced the older Pooled Cohort Equations) to determine your optimal LDL and non-HDL targets. Non-HDL cholesterol equals your total cholesterol minus your HDL — it captures all the harmful lipoproteins, not just LDL.

Important for South Asian and High-Risk Populations:

If you are of South Asian ancestry or have other high-risk factors (family history of premature CVD, elevated Lp(a), obesity, PCOS, gestational diabetes), we may recommend LDL targets below standard guidelines.

Why?

  • South Asians develop heart disease 5-10 years earlier than other populations
  • Higher rates of metabolic dysfunction even at "normal" cholesterol levels
  • Greater cardiovascular risk at lower LDL thresholds
  • Increased prevalence of small, dense LDL particles (more harmful)

Action: If you're South Asian, Hispanic, or African American and age 35+, your target is LDL < 55 mg/dL (not < 70 mg/dL).

This lower threshold (55 vs 70 mg/dL) applies even if you don't have diabetes or heart disease yet. Talk to your Porter Health coach about lifestyle intervention or medication to reach this target.

Statin Therapy: The Gold Standard

Statins are the most effective and well-studied medications for lowering LDL cholesterol. They've been proven to reduce heart attacks and strokes by 25-35%.

How Statins Work:

Statins block an enzyme (HMG-CoA reductase) in your liver that makes cholesterol. This forces your liver to pull cholesterol out of your blood, lowering LDL by 30-50%.

Statins also stabilize plaque, reduce inflammation, and improve blood vessel function - benefits beyond just lowering cholesterol.

Statin Intensity

Statins are classified by how much they lower LDL:

High-Intensity Statins (Lower LDL by ≥ 50%)

Used for very high risk patients (heart disease, diabetes with risk factors, LDL ≥ 190).

  • Atorvastatin (Lipitor) 40-80 mg
  • Rosuvastatin (Crestor) 20-40 mg

Moderate-Intensity Statins (Lower LDL by 30-50%)

Used for moderate risk patients or those who can't tolerate high-intensity.

Low-Intensity Statins (Lower LDL by < 30%)

Rarely used, mainly for those very sensitive to statins.

  • Simvastatin 10 mg
  • Pravastatin 10-20 mg
  • Lovastatin 20 mg

Common Side Effects

Muscle Pain (5-10% of people)

Most common side effect. Usually mild aches or soreness. Report severe pain or weakness immediately.

What to do: Try a different statin, lower dose, or CoQ10 supplement (100-200 mg daily).

Elevated Liver Enzymes (1-3%)

Usually mild and reversible. Your doctor will check liver tests before starting and periodically.

Increased Blood Sugar (Small increase)

May slightly increase diabetes risk in those already at risk. Benefits outweigh this small risk for most people.

Memory Issues (Rare)

Some people report memory problems, but large studies haven't confirmed this. Reversible if it occurs.

Common Statin Myths Debunked

Myth: "Statins damage your liver"

Truth: Serious liver damage is extremely rare (1 in 100,000). Mild enzyme elevation is common but not harmful.

Myth: "Everyone gets muscle pain"

Truth: Only 5-10% experience muscle aches, and many can tolerate a different statin or lower dose.

Myth: "I can stop once my cholesterol is normal"

Truth: Cholesterol will rise again if you stop. The medication is controlling it, not curing it.

Non-Statin Therapies

If statins alone don't lower your LDL enough, or if you can't tolerate them, these medications can help:

Ezetimibe (Zetia)

How it works: Blocks cholesterol absorption in your intestines.

Effect: Lowers LDL by 15-20%. When added to a statin, can lower LDL by up to 65% total.

Benefits: Very well-tolerated, minimal side effects. Often combined with statins for high-risk patients.

PCSK9 Inhibitors (Evolocumab, Alirocumab)

How it works: Monoclonal antibodies that help your liver remove more LDL from blood.

Effect: Lowers LDL by 50-60%. Dramatically reduces heart attack and stroke risk.

Who needs it: Very high risk patients who can't reach LDL goal with statins + ezetimibe.

How given: Self-injection every 2 weeks or monthly. Very safe and effective, but expensive (insurance coverage improving).

Bempedoic Acid (Nexletol)

How it works: Similar to statins but works only in the liver (not muscles).

Effect: Lowers LDL by 15-25%.

Benefits: Good alternative if you can't tolerate statins due to muscle pain. Oral pill taken once daily.

Inclisiran (Leqvio)

How it works: Uses RNA interference to reduce PCSK9 production in the liver.

Effect: Lowers LDL by 50%.

How given: Injection every 6 months (after initial loading doses). Very convenient for long-term management.

Dietary Strategies to Lower LDL

Diet can lower LDL by 5-10% on average, and up to 30% with aggressive dietary changes:

Reduce Saturated Fat

Effect: Lowers LDL by 5-10%

  • Limit red meat, butter, cheese, full-fat dairy
  • Choose lean protein (chicken, fish, beans)
  • Aim for < 5-6% of calories from saturated fat

Eliminate Trans Fats

Effect: Lowers LDL by 1-3%

  • Avoid partially hydrogenated oils
  • No fried foods, baked goods, margarine
  • Check labels for "trans fat" or "partially hydrogenated"

Increase Soluble Fiber

Effect: Lowers LDL by 5-10%

  • Oats, barley, beans, lentils (5-10g soluble fiber daily)
  • Fruits: apples, oranges, berries
  • Vegetables: Brussels sprouts, sweet potatoes

Add Plant Sterols/Stanols
